Authorization: This document serves to authorize the use of specialized cleaning agents, including EaCo Chem OneRestore cleaner , for the treatment of the client’s glass.. .
Condition of Glass: The undersigned client acknowledges and understands that there is a problem with the glass surfaces requiring special cleaning agents for restoration, and that the glass may be in a state of stage two corrosion. .
Risks: The undersigned client acknowledges that using specialized cleaning agents carries certain risks, and that damage to the glass may occur during the cleaning process. .
Release of Liability: In consideration for the use of EaCo Chem OneRestore, the undersigned client hereby releases and holds harmless “YOUR COMPANY NAME” and its agents from any and all claims, demands, or liability for damages that may arise from the cleaning of the glass using EaCo Chem OneRestore
