New Customer Survey thread

Making a new customer survey and was curious what you guys ask your customers.

Right now I have 5 exellent to poor questions for overall service, on-site employees.
Those are just to keep standards up

The fill-in the blank is where the meat is. Hoping to get some better questions than I already have.

What did you like about us?

What would you change?

Why do you like having a professional clean your windows?

This is something I have been toying with. So how/when do you give them the survey?

Ideas:

Will you refer us to your friends? Y/N (Not would you - will you, create a mental obligation lol)
Rate the quality of our work 1-5
Rate the friendliness of our staff
Rate the professionalism of our staff
Rate the knowledge of our staff
Did we answer all of your questions / concerns?
How would you rate the value of our services?
How would you rate the price of our services?
On a scale of 1-5 how much do you like peanut butter?

Not a bad list. Though personally I hate peanut butter. Its just gross.

Its going it our thank you packet. Included is a review page with web addresses and QR codes to rate us on Yelp, google, angies list. Also have hand written thank card, its the little things. Once I get them made a plastic gift card. Finally the questionare and pre-envelope and that already has a stamp.

I kinda feel like more than 3-5 questions will get diminishing returns… probably the simpler the better. Maybe some of these questions could be combined.

I have to agree, a few good straight to the point questions would be ideal.

If you had to limit your pick to only 5 questions - what questions would you have / use?
